MyMedicare
Registration Information and FAQ
My Medicare is a new voluntary patient registration model. It aims to formalise the relationship between patients, their general practice, general practitioner and primary care teams.

Registration is voluntary and free. When you register in MyMedicare it will notify us, your general practice, that you have chosen to nominate us as your regular care team. MyMedicare aims to provide greater continuity of care with your registered practice and improves health outcomes.

Registering with MyMedicare provides benefits to patients, general practices, and healthcare providers. For further information on these benefits and how to register, see below.

What benefits are there for patients?

Registering with MyMedicare provides greater continuity of care with your registered practice

You will be able to access longer telehealth consultations with your GP (>20 minutes)

From 1st July 2025 patients on Chronic Disease Management Plans will need to be linked with Hughes Clinic on MyMedicare to continue to be managed under a Chronic Disease Management Plan ("Care Plan")

From 1st November 2025 patients linked with a practice on MyMedicare will be able to access telehealth services even if they have not seen their GP face-to-face in the previous 12 months. Previously patients have been required to have at least one face-to-face consult in a 12 month period to access telehealth services.

How do I register with MyMedicare?

You're eligible to register in MyMedicare if you have a Medicare Card or DVA Veteran Card and you have had at least two face-to-face visits at our practice in the past 24 months

You can register for MyMedicare in your Medicare Online Account or Express Plus Medicare Mobile App. Alternatively you can fill out a registration form at our practice and we can complete the registration online for you (form available below).
Registrations can be withdrawn or changed at any stage and registering with our practice will not prevent you from accessing care from other practices or healthcare providers.
