Chronic Disease Management
We are committed to managing your chronic illness or complex health needs. We can arrange a GP Chronic Condition Management Plan, which helps to coordinate your care with your multidisciplinary team and a may entitle you to five Medicare subsided visits with an allied health professional each calendar year. Our clinic and doctors are committed to managing your chronic illness and complex care needs under a GP Chronic Condition Management Plan (GPCCMP). Our Chronic Disease Management Nurse will collaborate with your GP to ensure you have the best support, services and health outcomes. A GP Chronic Condition Management Plan will entitle you to 5 Medicare subsidised visits a calendar year to allied health services (e.g. podiatry, physiotherapy, exercise physiology). Care Plans have many advantages for you and allow us to address and manage your chronic health condition appropriately with ample allocated time.

Fees:
The preparation and review of your Care Plan are bulk billed to Medicare, meaning there is no additional out of pocket fees for this service.
